As our Associate Director of Population Health, you’ll develop and execute strategies to improve Value Based Care (VBC) results while leading a dedicated team working collaboratively with leadership and our providers to improve health outcomes.
Selected Responsibilities
• Deliver and execute strategic plan to create, develop, and maintain an Avance Care population health platform that addresses clinical and financial improvement across all relevant care categories including coordination with wellness, home, hospital, and specialty care.
• Ongoing management of Avance Care’s needs of the ACO platforms, including customizations.
• Develop tracking tools and metrics for monitoring progress for each ACO contract and across the portfolio of contracts to translate contract terms into actionable approaches to maximize ACO payments
• Evaluate actual results against expected performance and communicate areas for improvement
• Oversee fulfillment of reporting requirements of all payers, including CMS, and other governmental agencies
• Accountable for functional area budget and expense management activities through effective planning, monitoring and forecasting
• Responsible for overseeing the employee life cycle functions for direct reports staff including attraction, development, engagement, motivation, and retention
The ideal candidate will bring at least a minimum of 5 years of relevant experience in population health management analytics that with a track record of driving actionable insights for VBC success. Along with a bachelor’s degree is required, but master’s degree, or other advanced degrees in health services research, health policy, mathematics/statistics, or other relevant fields is highly preferred.

• Ability to perform analytical problem solving as well as project management
• Knowledge of and the ability to rapidly learn about drivers of VBC success in HEDIS/STARS quality metrics, episode grouper methodologies, high value specialist ranking methodologies, HCC/RAF coding, Behavioral Health integration, nutrition, chronic care management, and other drivers of VBC success
