You are someone with a good understanding of adult community care activities and experience in a social care setting. You have a strong educational background including literacy, numeracy, and IT skills, with relevant qualifications or a willingness to obtain them. What the job involves

This role involves undertaking assessments and reviews to assist in maintaining the independence of adult social care service users. You will complete assessment forms, manage records, and coordinate support packages. Your responsibilities include identifying resources, co-ordinating care, conducting home visits, designing and arranging equipment delivery, managing expenditure, liaising with agencies, and ensuring best value through close monitoring. You will handle safeguarding concerns, respond to queries, develop knowledge in communication needs, participate in training, and adhere to council policies.
