This idea is more than a slogan; it is our promise to patients, their families, and every staff member that each day is worth celebrating.Responsibilities:As the Licensed Practical Nurse (LPN), you will play a vital role in coordinating patient care and supporting our team of healthcare professionals who provide medical services in patients' homes and community settings.
- Working from our office, you will serve as a primary point of communication for patients, families, caregivers, community partners, and healthcare providers, ensuring questions and concerns are addressed promptly and appropriately.
The LPN works closely with our Nurse Practitioners and other clinical team members to coordinate patient visits, prioritize care needs, process referrals, and facilitate communication between all members of the care team.
Responsibilities include documenting patient information in the electronic medical record, processing medication requests and refills as directed, coordinating laboratory services, durable medical equipment (DME), and other clinical resources, faxing and tracking provider orders, providing patient and caregiver education, assisting with care coordination, and supporting the timely completion of clinical documentation.

Experience in hospice is also preferred.Current First Aid and CPR certificationSchedule flexibility and local travel requiredAbility to communicate effectively verbally and in writingMust have valid driver's license, vehicle insurance, and reliable transportation
