As a Hospice LVN, you’ll work closely with our RNs, physicians, and interdisciplinary care team to provide skilled nursing care, monitor patient symptoms and changes in condition, assist with medication and symptom management, and provide education and support to patients and their families.

• PRIMARY FUNCTION

Anthem Hospice provides services to its patients and their families 24 hours a day, 7 days a week. The Hospice Licensed Practical Nurse (LPN) is responsible for administering nursing care according to the plan of care to terminally ill patients admitted under the direction of a Registered Nurse (R.N.), in accordance with the Nurse Practice Act in the State where the Hospice program is located.

A Licensed Practical Nurse Auditor is responsible for monitoring charts to ensure compliance in accordance with Federal, State and local regulations.

• JOB RESPONSIBILITIES
• Provides direct skilled bedside nursing care within the LPN scope of practice in accordance with the patient/family Plan of Care.
• Under the supervision of the R.N.:
• Evaluates patient/family needs and assists in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
• Performs selected acts in the care of the terminally ill which may include the administration of treatments and medications.
• Evaluates effectiveness of treatments and monitors patient’s reactions.
• Maintains a clean and safe environment.
• Maintains accurate clinical notes on interdisciplinary charts.
• Reports significant findings or changes in the patient’s condition to the R.N., Team Manager or Physician. Documents reports in clinical notes.
• 6. Routinely monitors both active and closed charts to assure that services are rendered as integrated components of the interdisciplinary plan of care, that the team makes decisions which are appropriate and responsive to patients’/families’ needs and expectations 24 hours a day, 7 days a week, and that documentation effectively demonstrates quality and regulatory compliance.
• Collaborates with Nurse Manager(s) and total program in developing Performance Improvement Plans.
• Collaborates with Nurse Manage to prepare for survey process and annual program review, assuring that company policies, procedures, and Federal, State and local regulations are followed.
• Collaborates with Nurse Manager to compile and review data for quarterly QAPI studies.
• JOB REQUIREMENT

Must have and maintain transportation to be used for work with required liability insurance and license.

• QUALIFICATIONS
• Graduate of an approved school of Practical/Vocational nursing.
• Currently licensed to practice nursing in the state where the Hospice program is located.
• At least one year, preferably two years of satisfactory nursing experience, at least one year of which is as an LPN/LVN in a hospital or skilled nursing facility.
