Ambulatory Oncology Social Worker

Provide psychosocial support, care coordination, and discharge planning services to patients and families. Collaborate with the interdisciplinary team to address social, emotional, and environmental factors that impact patient care and outcomes.

Education

Master's Degree in Social Work (MSW) from a CSWE-accredited program

Preferred Qualifications

Clinical licensure or provisional licensure
Accredited Case Manager Certification (ACM-SW)
Healthcare social work experience

Key Responsibilities

Assess patients' psychosocial and discharge planning needs.
Develop and implement individualized treatment and care plans.
Provide crisis intervention, counseling, and family support.
Coordinate internal and external referrals and community resources.
Advocate for patients and families to address barriers to care.
Collaborate with the healthcare team on discharge planning and transitions of care.
Coordinate care conferences as needed.
Maintain knowledge of regulations, payer requirements, and reimbursement practices.
Document assessments, interventions, and care coordination activities in the EMR.
