Join our dedicated team as an Afterhours RN Care Manager, where you will provide high-quality, compassionate hospice care during evenings, weekends, and on-call hours. This position is vital for ensuring continuity of care and clinical management for patients and families outside of standard business hours.
About the Role
As an Afterhours Care Manager RN, you will assess and coordinate the needs of patients and families during your shifts, providing primary clinical oversight for your assigned cases. Your responsibilities will include routine visits, admissions, death visits, urgent after-hours calls, and any other visits specified in the patient's care plan. You will collaborate actively within the Interdisciplinary Group (IDG) to promote effective communication among physicians, families, and the hospice team, while also engaging in clinical administrative tasks to ensure compliance and quality assurance.
Essential Functions
• Facilitate the proper delivery of hospice services to eligible patients and their families.
• Provide hands-on patient care during your shifts, including assessments, admissions, and other assigned visits.
• Conduct thorough assessments addressing physical, emotional, psychosocial, and spiritual needs.
• Support the IDG process through chart reviews and collaboration on care plans.
• Help develop and update the Hospice Plan of Care alongside physicians and IDG members.
• Educate patients and families about disease progression and symptom management.
• Communicate any significant changes in patient conditions to attending physicians promptly.
• Maintain accurate documentation of clinical assessments, care plans, and nursing notes.
• Participate in scheduling modifications based on clinical needs.
• Support compliance and administrative tasks, ensuring quality in documentation and workflow processes.
• Assist with regulatory compliance and quality improvement initiatives.
• Uphold infection control standards and best practices consistently.
• Provide guidance and support to field staff and conduct supervisory visits when necessary.
• Engage in team meetings and training programs as required.
• Continue to develop expertise in pain and symptom management for end-of-life care.

• Perform additional duties as assigned.
About You:
Education and Experience
• Current RN license in the state of practice.
• Graduate from an accredited nursing program.
• At least one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Compliance with any state-specific requirements.
Licenses and Certifications
• Active RN license.
• CPR certification (in-person).
• Valid driver's license, reliable transportation, and active auto insurance.
Skills and Competencies
• In-depth knowledge of hospice principles and Medicare/Medicaid regulations.
• Proficient in conducting patient assessments and creating individualized care plans.
• Effective communication skills with diverse groups including patients, families, and interdisciplinary teams.
• Strong time-management abilities in a dynamic environment.
• Proficiency in EMR systems (preferably HomeCare HomeBase), Microsoft Office, and mobile solutions.
• Integrity in maintaining confidentiality and supporting quality healthcare outcomes.
• Leadership capabilities to mentor and assist field staff.
• Accurate management of documentation and compliance responsibilities.
