We are hiring a Float Advanced Practice Provider (NP/PA) to support a large regional footprint — providing coverage for PTO, turnover, and new building launches while maintaining continuity of high-quality, relationship-based care.

This is a high-visibility role ideal for experienced clinicians who are adaptable, operationally strong, and interested in future leadership opportunities.

What You’ll Do

Provide mobile primary care to seniors in homes, boarding cares, ALF, SNF, and hospice settings

Step into established panels to ensure seamless patient continuity

Support new market expansion and building launches

Manage chronic conditions (HTN, CHF, COPD, diabetes, dementia, polypharmacy)

Deliver dementia-informed, relationship-based care

Collaborate with interdisciplinary partners (psychiatry, palliative care, neurology)

Maintain strong documentation, communication, and quality standards

What We’re Looking For

Active, unencumbered Nurse Practitioner (NP) or Physician Assistant (PA) license

2+ years experience in geriatrics, primary care, home health, SNF, or senior living preferred

Comfortable practicing independently in mobile/community settings

Strong clinical judgment and panel management skills

Adaptable, organized, and growth-oriented

Valid driver’s license and ability to travel within region
